Five Hole Hockey Team Rules and Code of Conduct
Rules and Code of Conduct
- Respect your teammates, coaches, officials, Opponents, and Parents-No horseplay, bullying or disrespectful behavior will be tolerated.
- Arrival Time: Players are expected to be on time for all team events. Please plan accordingly for travel, etc. Players are expected to be at the rink 1 hour prior to game time for all games.
- Game Day Dress Code: Players should dress in their Team Five Hole polo shirt and dress pants or shorts and their Team Five Hole work out shirt and shorts for pre-game warmups.
- Locker Rooms will be monitored and access will be restricted. Players, coaches, and Team manager are the only people allowed in the locker rooms. No exceptions.
- No cell phones or recording devices in the locker room. The only exception is that a coach or approved player can use one to play music
- No picture taking, or video under any circumstances by anyone is permitted inside the locker rooms.
- Horseplay of any kind is not permitted.
- If parent assistance is needed, the player must leave the locker room.
- Players Benches, Penalty Boxes, Ice Surface and Rink:
- Only players, coaches and team manager are allowed in/on the ice benches, penalty boxes, and locker rooms. No exceptions.
- In the rink/stands please let the coaches teach the game and the kids play it. Please refrain from shouting or signaling 'instructions" to players while on the ice. This is very counterproductive to player and team development and does more harm than good.
- If a player or parent is expelled from the rink and or playing in games by the referees, tournament staff or coaching staff or asked to leave the hotel for any reason. We may ask you not to participate in this and/or any future Five Hole Hockey games, camps, and events, no refunds will be given. We expect everyone to behave in a positive and respectful manner at all times as you are representing yourself, your child, and Five Hole Hockey.
- All players must at all times on the ice wear full gear, a mouth guard and neck guard as per USA Hockey rules. Any violation and you will be asked to leave the ice until you have proper equipment, you will not be permitted back on the ice until you do so.
- Players will not drink alcohol or use any type of performance enhancing drugs or illegal drugs of any kind. Doing so will result in immediate removal from the team/camp with all fees paid forfeited.
- A Player with any signs/symptoms of a concussion will be removed from play and will need a doctor's note to return to plaly.
- Issue Resolution: Should an issue arise, if you have an issue bring it to the attention of Andy or Jaime Holmes.
- Staff and Five Hole Hockey may make changes to these rules at any time with no warning.
I agree to follow all the rules and code of conduct at all Five Hole Hockey Tournaments, camps and events and I understand that violating any of these rules I may be asked to not participate in this/and any other Five Hole hockey tournaments, camps, and events and will forfeit any monies paid.
